If you are using version 7.6.x you can also use an app called HFM.

I've seen that a few times myself. I have no idea what causes it, but usually it's not taking place quickly but more towards the the middle to end of the timeout time.

I believe I have only seen clock skew from the system sleeping. Besides a system sleeping, I have seen clock skew reported on a machine that had I/O stall.
